New Hampshire Motor Vehicle Power of Attorney Form (Title 5)

The New Hampshire Motor Vehicle Power of Attorney (Title 5) is a basic form that makes it easy for a vehicle owner to appoint another party to be their lawful attorney. This party will be granted the authority to apply for Certificate of Title or Registration of the vehicle that is the subject of the Power of Attorney. In order for the form to be considered valid, and therefore filed with the relevant authority, it must be:

  • Signed by the vehicle owner, and
  • Notarized by a Notary or Justice of Peace.
