Minnesota Motor Vehicle (DMV) Power of Attorney Form
The Minnesota Motor Vehicle Power of Attorney Form is a form an individual may file with the Minnesota Department of Public Safety, Driver and Vehicle Services to request for a Power of Attorney be put into effect in relation to a given motor vehicle. Via the form, the motor vehicle owner will need to provide relevant details about the parties involved. Moreover, they will need to provide information about their motor vehicle, namely, its:
- Body Type,
- Year, and
- Vehicle Identification Number (VIN).

Signing Requirements – Neither state law nor the Minnesota Department of Public Safety, Driver and Vehicle Services dictate any particular signing requirements. However, to increase the chance that the Power of Attorney will be considered valid by the Department, it is strongly recommended that the document is signed acknowledged by a Notary Public and is witnessed by two (2) witnesses.
Unlike most states, the Minnesota Department of Public Safety, Driver and Vehicle Services does not provide motor vehicle owners with an official Motor Vehicle (DMV) Power of Attorney form. As such, individual wishing to establish this POA will need to create their own form or use a standard document template, as provided above.
