Louisiana Tax Power of Attorney (Form R-7006)

The Louisiana Tax Power of Attorney (Form R-7006) is a form issued by the Louisiana Department of Revenue that makes it possible for a taxpayer (or a taxpayer and their spouse) to appoint a Representative who will act as their “true and lawful” Agent and Attorney-in-Fact before the Louisiana Department of Revenue. The Representative will have the authority to complete specific acts in relation to specific tax matters that the taxpayer/s have noted down in the form.
