Kentucky Minor Child Power of Attorney Form (AOC-796)

The Kentucky Minor Child Power of Attorney (AOC-796) is a form that allows a parent or legal guardian to elect an Attorney in Fact who will act in their place and stead to administer care to their child in the contexts provided for. The parent or legal guardian should use the form to clearly define the extent they wish the Attorney in Fact to exercise their decision making powers. For instance, they may delegate powers for the ‘Attorney in Fact’ to act in regard to their child’s medical treatments, but not their schooling (or vice versa).
