Indiana State Tax Power of Attorney (Form 49357)


A two-page document issued by the Indiana Department of Revenue, the Indiana State Tax Power of Attorney (Form 49357) provides a taxpayer with the opportunity to authorize a Representative to act on their behalf when performing specified or general tax-related duties. The taxpayer must complete a number of mandatory sections for this request to be processed, such as providing information about themselves as well as their Representative as well as identification numbers. They must also specify the tax matters and tax period they wish the Power of Attorney to be put into effect.

Signing Requirements (IC 30-5-4-1) – The Indiana Department of Revenue states that the only signing requirement for Form 49357 is that the taxpayer signs the form.
