Indiana Motor Vehicle Power of Attorney (Form 1940)
The Indiana Motor Vehicle Power of Attorney (Form 1940 Rev. 5-13) is a document that allows an individual to designate an Attorney-in-Fact to complete transactions involving the certificate of title and/or registration for the vehicle or watercraft they name in the form. As per the Indiana Bureau of Motor Vehicles’ stipulations, the designation will only be valid for ninety (90) days from the date of notarization. Anyone completing this form must comply with the guidelines the Bureau has stated at the top of it in order to ensure its validity and subsequent execution.
