Arizona Minor Child (Parental) Power of Attorney Form
The Arizona Minor Child (Parental) Power of Attorney provides parents or guardians with the legal scope to nominate an Agent to assume custody of their child in the circumstances they note in this contract. This limited type of Power of Attorney is constrained by a state-mandated maximum time frame. That is, § 14-5104 stipulates that the delegation of parental of guardianship powers must not exceed a period of six (6) months.

Signing Requirements (§ 26-1A-105) – Acknowledgment by the Principal before a Notary Public is necessary for an Arizona Minor Child (Parental) Power of Attorney to be considered valid. Moreover, one (1) witness must be present at the time the Principal signs the form.
